Here are two scenarios that I've found that appear to be similar in nature.

If a customer uses a discount code for an order, completes it and comes back to the store to buy more: the discount still gets applied to one product.

- OR -

If a customer uses a discount code for an order, and removes the discount code before checkout: the discount still gets applied to one product.

This was actually due to user error on my part. It has since been fixed.
